Verification Requirements:

Domestic Graduates:

1 -Medical School Transcripts

2 -Medical School Form

3 -All Internship/Residency/Fellowships completed in the USA/Canada/Puerto Rico

4 -All State Physician Licenses

5 -All National and State Board Examinations

International Graduates:

6 -ECFMG CVS Report (Foreign Medical School Graduates)

If Licensed for 10 years or more in one of the 50 States, then Reciprocity is extended. The Physician only has to verify past and current licensure history.

Educational, Examination, and Training History Requirements:

- 2 years of PGT in an approved program

- ECFMG Certification for Foreign Medical Graduates

- Must pass USMLE Step 3 within a 5 year time frame from first attempt of Step 3.
